Information you provide. When you contact us or submit a form (such as an appointment request), we collect the information you choose to provide, such as your name, phone number, email address, and the reason for your inquiry.

Forms and health information
To protect your privacy, please do not submit detailed health information through general website channels. Our appointment request form deliberately asks only for your name, email address, phone number, and a short message, and it tells you not to include medical details — the form is not a medical record system, and information sent through it is not covered by the protections that apply to your medical record. Submissions are transmitted over an encrypted connection and handled by our website host's form service, so they do not reside on the marketing website itself. If you need to discuss health details, please call the office instead. Health information you share with us as a patient is handled in accordance with our Notice of Privacy Practices and applicable law.
Cookies and analytics
We use Google Analytics 4, a web analytics service provided by Google LLC, to understand how visitors find and use the Site so that we can improve it.
Google Analytics sets cookies and similar identifiers that collect standard usage information — the pages you view, how long you spend on them, the links you follow, your approximate location (derived from your IP address, typically no more precise than a city), your device and browser type, and whether you arrived from a search engine, another website, or directly. Google anonymizes IP addresses collected through Google Analytics 4 and does not store them.
