If your plan requires a referral, our team will let you know and help you gather what is needed. Referring physicians can also send patients to us directly.

If you have prior imaging, test results, or records related to your vascular condition, bring those as well, or ask your previous provider to send them ahead. If you submit near a weekend or holiday, you may hear from us the next business day. For an urgent medical need, please call 911 rather than waiting for a response.
